These Terms of Service ("Terms") govern your access to and use of the CliniForm platform and related services (the "Service") provided by CliniForm ("we", "us", or "our").
By creating an account, subscribing, or using the Service, you agree to these Terms. If you do not agree, you must not use the Service.

1. Definitions
- "You" or "Customer" means the individual or organisation that registers for an account;
- "Practice" means your business (clinic, salon, or sole practice) using the Service;
- "End client" means an individual who completes a consultation form hosted through your account;
- "Content" means data, forms, notes, and materials you upload or create using the Service.
2. The Service
CliniForm provides a cloud-based platform for wellness and therapy practices to create consultation forms, collect digital signatures, manage client records, and related practice management features.
We may update, modify, or discontinue features from time to time. We will use reasonable efforts to notify you of material changes that adversely affect core functionality.
3. Account registration
You must:
- Be at least 18 years old and have authority to bind your practice;
- Provide accurate and complete registration information;
- Keep your login credentials confidential;
- Notify us promptly of any unauthorised access to your account.
You are responsible for all activity under your account, including actions by staff members you invite.
4. Subscriptions and payment
Access to certain features requires a paid subscription. Pricing is displayed on our website and may change with reasonable notice.
- Subscriptions are billed in advance on a monthly basis unless otherwise stated;
- Payments are processed securely via Stripe;
- Free trials may be offered at our discretion and convert to paid plans unless cancelled;
- You may upgrade or downgrade your plan through your account billing settings;
- Unless required by law, fees are non-refundable once a billing period has commenced.
Failure to pay may result in suspension or termination of your account.
5. Acceptable use
You agree not to:
- Use the Service for any unlawful purpose or in breach of applicable regulations (including health, data protection, and consumer protection law);
- Upload malicious code, attempt unauthorised access, or interfere with the Service;
- Use the Service to send spam or unsolicited communications;
- Misrepresent your identity or affiliation;
- Resell or sublicense the Service without our written consent;
- Collect or process special category data without an appropriate lawful basis and safeguards.
We may suspend or terminate accounts that violate these Terms.
6. Your content and data
You retain ownership of Content you submit. You grant us a limited licence to host, store, process, and display Content solely to provide the Service.
You are responsible for:
- Ensuring you have the right to collect and process end client data;
- Providing end clients with appropriate privacy information;
- The accuracy and legality of forms and content you publish;
- Backing up critical data where required for your professional obligations.
7. Data protection
Each party shall comply with applicable data protection law. Where we process personal data on your behalf, we act as a data processor and you act as the data controller for end client data.

8. Intellectual property
The Service, including software, design, branding, and documentation, is owned by CliniForm or its licensors. These Terms do not grant you any rights to our intellectual property except the limited right to use the Service as permitted herein.
You may not copy, modify, reverse engineer, or create derivative works of the Service except as permitted by law.
9. Third-party services
The Service may integrate with third-party providers (such as Stripe for payments). Your use of third-party services is subject to their own terms and policies.
10. Availability and support
We aim to provide reliable access to the Service but do not guarantee uninterrupted or error-free operation. Planned maintenance will be communicated where practicable.
Support is provided via email during standard UK business hours. Response times may vary by subscription tier.
11. Disclaimer
The Service is provided for administrative and record-keeping purposes. It does not provide medical, legal, or professional advice. You remain solely responsible for clinical decisions, treatment, and compliance with professional standards.
AI-assisted features, where available, are informational tools only and must not replace your professional judgement.
12. Limitation of liability
Nothing in these Terms excludes or limits liability that cannot be excluded under applicable law, including liability for death or personal injury caused by negligence, or fraud.
Subject to the above:
- We are not liable for indirect, incidental, or consequential losses (including loss of profit, data, or goodwill);
- Our total liability arising from or related to the Service in any 12-month period is limited to the fees paid by you in that period, or £100 if you are on a free plan.
13. Indemnity
You agree to indemnify us against claims, losses, and expenses arising from your breach of these Terms, your Content, or your processing of end client data.
14. Term and termination
These Terms commence when you create an account and continue until terminated.
You may cancel your subscription at any time through your account settings. Cancellation takes effect at the end of the current billing period.
We may suspend or terminate your account immediately if you materially breach these Terms, fail to pay fees, or if required by law.
Upon termination, your right to access the Service ceases. We may delete your data after a reasonable retention period, subject to legal obligations.
15. Changes to these Terms
We may update these Terms from time to time. We will post the revised version on this page and update the "Last updated" date. Continued use of the Service after changes take effect constitutes acceptance of the revised Terms.
16. Governing law
These Terms are governed by the laws of England and Wales. The courts of England and Wales shall have exclusive jurisdiction, except where mandatory consumer protection laws in Scotland or Northern Ireland apply.
