Walkthrough

  1. Go to Form templates in the left menu.
  2. Create a template with sections, questions and required fields.
  3. Assign templates to services or send manually from a client profile.
  4. Clients can complete forms via the portal link or on a tablet in reception.
  5. Submitted forms are stored on the client record and can be exported.

Tips

  • Use conditional logic to show follow-up questions only when relevant.
  • Duplicate an existing template to speed up creating variants.
